BHcare and Fair Haven Community Health Care are proud to collaborate on Shoreline Family Health Care’s innovative care model, each bringing their unique expertise to benefit the Shoreline community.

BHcare is a community-based behavioral health organization with nearly 40 years of experience providing comprehensive mental health, addiction and domestic violence services for adults and children throughout the Shoreline, Greater New Haven and the Lower Naugatuck Valley.  BHcare is designated by the Department of Mental Health and Addiction Services as the Local Mental Health Authority for six Shoreline towns, and five Valley towns.  BHcare is licensed by the Department of Public Health and accredited by The Joint Commission.

Fair Haven Community Health Care has been making high quality, patient-centered health care accessible to the communities they serve for almost 50 years. Since 1971, Fair Haven Community Health Care has provided a medical home to nearly 18,000 adults and children in the Greater New Haven community. Patient care includes a wide range of primary and specialty care services, as well as evidence-based patient programs to educate patients in healthy lifestyle choices,  pre-natal care and parenting workshops. Fair Haven Community Health Care is licensed by the CT Department of Health and accredited by The Joint Commission and is designated a gold Level Health Center Quality Leader by HRSA and recognized as a Patient-Centered Medical Home by NCQA.

The Shoreline Family Health Care integrated care model provides patients access to the expertise of both organizations, in one visit.
