What is MyChart?

MyChart, Shoreline Family Health Care’s patient portal, provides easy and secure access to your medical information on-line. You can view your personal health records wherever you have access to an internet-linked computer or smart phone. You can also download the MyChart app for Apple and Android devices.

If it’s your first time, click here to sign up with your MyChart Access Code. Your signup access code will appear on the last page of your after- visit summary. Please call your primary health care provider (PCP) if you did not receive your after-visit summary.
