You may be wondering whether you must meet certain requirements to get medical benefits. The requirements of medical plans vary, depending on the company providing the benefits. Not all plans require an examination; however, there are sometimes minimum requirements to qualify. For example, state-administered health care plans, such as Medicaid, often do not rely on medical testing, but rather use income guidelines to determine your qualifications. Other private health care plans may require basic physical examinations to ensure you qualify for certain medical plans. New laws have been established to prohibit plan administrators from discriminating against patients/consumers based on pre-existing medical conditions, such as diabetes or hypertension. Therefore, it is important that you address any concerns or questions to the proper channels for any health care plan you are interested in applying for. They will help you apply for health insurance or determine if you qualify for our Sliding Discount Fee Program.
For your first appointment at Shoreline Family Health Care, you will need to provide photo identification to register. We also request that you bring a list of your current medications, immunizations, or past health records, if available, as well as your health insurance card, if applicable. When you make your first appointment, we will preregister you by phone. Please also bring a method of payment as required for your visit. We accept most private health insurance and managed care plans, as well as Medicaid/HUSKY and Medicare. If you are eligible for our Sliding Fee Discount Program, you can pay by cash, check, money order, or credit and debit cards. If you have a high co-pay for your visit and payment is a concern, we still want to help you get healthy and stay that way, thus one of our Certified Insurance Counselors may be able to assist you with our Sliding Discount Fee Program. If on the off chance you require a prescription that we don’t carry within our own pharmacy, we will ensure we electronically transmit your prescription when possible, thus limiting the time required for your trip to an outside pharmacy.
